Registration and obtaining medical license

Physician’s/Dentist’s medical license – limited to the graduate work placement/internship period:

• Graduate work placement – physicians and dentists with Polish citizenship
•  Graduate work placement – physicians and dentists with non-Polish citizenship

Physician’s/Dentist’s medical license – after graduate work placement/internship and LEP/LDEP exam:

•  Obtaining physician’s/dentist’s medical license in Poland

Physician’s/Dentist’s medical license – how to renew a medical license that is no longer valid:

• Registration onto the Regional Chamber’s members list and into the Regional Chamber’s Registry in case of a physician or a dentist who already has got a medical license obtained according to previous regulations and obtaining the physical document ‘Physician’s medical license’ or „Dentist’s medicla license’

Physician’s/Dentist’s medical license – other cases:

•  Obtaining your medical license’s duplicate
•  Indefinite discontinuation of medical practice
•  Renouncing one’s medical license
•  Reopening of medical practice after indefinite discontinuation or after dicontinuation which lasted longer than 5 years

Foreigners, citizens of EU member states:

• Obtaining physician’s/dentist’s medical license in Poland by citizens of EU member states

Non-EU citizens:

•  Obtaining physician’s/dentist’s medical license in Poland by non-EU citizens
•  Renewal of medical license in Poland by foreigners
•  Obtaining medical license by a foreign member of the Regional Chamber, who received Polish citizenship

Other matters concerning registration:

•  Switching membership to a different regional chamber of physicians and dentists
•  Updating your data/information in the Regional Chamber’s register of physicians and dentists
