Procedure overview:
Upon the request of a physician/dentist, an EU member state citizen, a regional chamber of physicians and dentists confers the medical license to practice in Poland, after consideration of opinion from Qualifications Recognition Centre of the Supreme Chamber of Physicians and Dentists.
Required documents:
1. Application W-2A – title: „Wniosek o uznanie kwalifikacji formalnych, przyznanie prawa wykonywania zawodu lekarza/lekarza dentysty, wpisanie na listę członków i wpis do rejestru okręgowej izby lekarskiej” – a request to recognise formal qualifications, confer medical license, enter into the register and onto the member list, which contains the applicant’s statement of full legal capacity and consent to process personal data contained in the regional register of physicians and dentists.
2. a diploma/certificate of graduation from a medical school/college (Medical Academy, University) – photocopy + translation into Polish by a sworn translator;
3. graduate work placement/internship – confirmation of completion if mandatory in the state of origin – photocopy + translation into Polish by a sworn translator;
4. medical license conferred by relevant authorised authority – Ministry of Health or Medical Chamber – photocopy + translation into Polish by a sworn translator;
5. Certificate of Polish language test organised by the Supreme Chamber of Physicians and Dentists, or submission of matriculation examination in the Polish language;
6. confirmation of work experience lasting at least 3 out of 5 previous years issued by relevant authorised authority – Ministry of Health or Medical Chamber – photocopy + translation into Polish by a sworn translator;
7. “Certificate of Good Standing” i.e. attestation of clean disciplinary record, issued by relevant authorised authority – Ministry of Health or Medical Chamber – photocopy + translation into Polish by a sworn translator;
8. Certificate of health, issued by a physician authorized on the basis of relevant regulations concerning preventive healthcare and occupational medicine (a catch document model is available on the Registration Committee’s site – see: “Formularze” field, or in the regional chamber’s office),
9. a document confirming citizenship – passport – photocopy
10. 2 photos – passport format
11. document confirming obtained specializations – photocopy + translation into Polish by a sworn translator;
12. personal data processing notice – signed statement

Response period:
One month. In more complex cases – no longer than 2 months from the submission of complete set of documents.
Appeal procedure:
Within 14 days of receipt of negative decision the applicant may submit an appeal. An appeal shall be submitted to the Supreme Medical Council, by way of regional medical council. The applicant may appeal a Supreme Medical Council’s decision to the Supreme Administrative Court.
