Procedure Overview

Regional Medical Council adopts a resolution conferring medical license to practice in Poland, in accordance with acquired citizenship.

Required documents

  1. An application form: „ Wniosek o przyznanie prawa wykonywania zawodu zgodnie z posiadanym obywatelstwem” i.e. a request to confer medical license in accordance with acquired citizenship.
  2. a decision of the President of the Republic of Poland granting Polish citizenship.
  3. ID card.
  4. 1 photo in passport format, along with digital version in jpg file format –  min. resolution: 420 x 525, max. file size: 200 KB.
  5. medical license document issued previously.
  6. personal data processing notice – signed statement

Appeal procedure

Within 14 days of receipt of negative decision the applicant may submit an appeal. An appeal shall be submitted to the Supreme Medical Council, by way of regional medical council. The applicant may appeal a Supreme Medical Council’s decision to the Supreme Administrative Court.
